How to: Create a form with Form Wizard using only one table or query.
Solution:
Click the 'Forms' tab in the Database window, click 'New', select 'Form Wizard', select a table or query, and follow the steps in the wizard.
1) If the Database window is not active, activate the Database window.
2) Click the 'Forms' tab in the Database window.

3) Click 'New'. (The New Form dialog box appears.)
4) Select 'Form Wizard' from the list box.
5) (Optional) Select the desired table or query from the 'Choose the table or query where the object's data comes from' drop-down list box.
NOTE 1: If no selection is made, the first object from the table/queries list is used.
NOTE 2: Use the scroll bar for more selections if necessary.

6) Click 'OK'. (The Form Wizard dialog box appears.)
7) (Optional) If a table or query was NOT selected in step 5), select the desired table or query from the 'Tables/Queries' drop-down list box in the 'Form Wizard' dialog box.
NOTE 1: If a table or query WAS selected in step 5), it is already selected in the 'Tables/Queries' box.
NOTE 2: Use the scroll bar for more selections if necessary.
Click Table/Queries from the Form Wizard.
8) Select the desired fields to include on the form:
a) Select a field from the 'Available Fields' list box.
b) Click '>'. (The field name appears in the Selected Fields list box.)
c) Repeat steps 8)a) and 8)b) for each field to include on the form.
Select the desired field.
9) Click 'Next'.
10) Select the desired radio button for the form layout. (A preview of the form appears to the left of the radio buttons.)

11) Click 'Next'.
12) Select the desired style for the form from the list box. (A preview of the style appears to the left.)
NOTE: Styles from which to choose include: Clouds, Colorful 1, Colorful 2, Dusk, Evergreen, Flax, International, Pattern, Standard, and Stone.
Select a style of the printed form.
13) Click 'Next'.
14) Type a title for the form in the 'What title do you want for your form?' box.
NOTE: If a title is not typed, the name of the table or query used to create the form is used as the form name.
15) Do one of the following:
a) Select the 'Open the form to view or type information' radio button to view the form or type data.
b) Select the 'Modify the form's design' radio button to open the form in the Design view.
Type a form title and select radio button.
16) Click 'Finish'. (The new form appears in either Form view or Design view, depending on the radio button selected in step 15).)
